
A major data breach at a US healthcare technology firm has impacted 3,433,965 individuals.
According to a breach notification filing, the incident at TriZetto Provider Solutions was the result of an external system breach involving unauthorized access to company systems.
TriZetto Provider Solutions is based in Missouri and provides claims processing and revenue management services to healthcare providers.
The breach happened on November 19, 2024, but was not discovered until November 28, 2025.
The company says the compromised data may include names and other personal identifiers combined with sensitive information such as Social Security numbers and health insurance information.
The breach affected at least 1,128 residents in the state of Maine, according to the filing submitted by attorney Edward Zacharias of the law firm McDermott Will & Schulte, which represents the company as counsel.
The breach notification filing indicates the incident involved hacking of an external system rather than an internal error or accidental disclosure.
TriZetto began notifying affected consumers in writing on February 6, 2026.
The company is offering impacted individuals 12 months of identity theft protection and single-bureau credit monitoring services through Kroll.
